General Liability Waiver and Release of Claims
NOTICE TO PARTICIPANTS AND PARENTS/GUARDIANS: THIS IS A LEGALLY BINDING AGREEMENT. PLEASE READ CAREFULLY BEFORE SIGNING.

Participant Information:
Assumption of Risk: I, the undersigned, acknowledge and understand that participation in activities at Legends Sports and Entertainment, including but not limited to basketball, baseball, obstacle courses, batting cages, trampolines, sand pits, and other open play areas, involves inherent risks. These risks include, but are not limited to, the risk of serious physical injury, death, property damage, or other harm, which may arise from the actions or negligence of myself, other participants, staff, or equipment failure.
I understand that these activities may involve physical exertion and interaction with other participants that could result in injury. I voluntarily assume all risks associated with participation in these activities, whether known or unknown, and agree that my participation is at my own risk.
Release of Liability: In consideration of being allowed access to the facilities and participation in activities at Legends Sports & Entertainment, I hereby release, discharge, and hold harmless Legends Sports & Entertainment, its owners, employees, agents, representatives, successors, and assigns ("Released Parties") from any and all claims, demands, causes of action, or liabilities, whether known or unknown, arising out of or connected with my participation in any activities or use of the facilities. This release applies to any injury, damage, or loss that I may sustain, including injuries caused by the negligence of the Released Parties.
Indemnification: I agree to indemnify, defend, and hold harmless the Released Parties from any claims, demands, or causes of action brought against them by any third party as a result of my participation in activities or use of the facilities. This includes any costs, expenses, or legal fees incurred by the Released Parties in defending such claims.
Responsibility for Property Damage: I understand and agree that I am responsible for any damage I may cause to the property, equipment, or items belonging to Legends Sports and Entertainment. This includes, but is not limited to, damage resulting from misuse, negligence, or intentional acts. I agree to reimburse Legends Sports and Entertainment for the full cost of repairs or replacement of any damaged property or items. If the damage is caused by a minor participant, I, as the parent or legal guardian, assume full responsibility for the costs of repairs or replacement.
Medical Authorization: In the event of an emergency, I authorize Legends Sports & Entertainment and its representatives to obtain medical treatment for me (or my minor child) as deemed necessary. I agree to assume all costs associated with such medical treatment and waive any claims against the Released Parties for such treatment or transportation to a medical facility.
Arkansas State Law Compliance: This waiver and release shall be governed by and interpreted in accordance with the laws of the State of Arkansas. I acknowledge that Arkansas law may limit liability waivers for certain types of negligence or intentional misconduct. To the extent any provision of this waiver is found to be unenforceable under Arkansas law, such provision shall be severed, and the remaining provisions shall remain in full force and effect.
Minor Participants: If the participant is under the age of 18, I, as the parent or legal guardian, agree to the terms of this waiver and release on behalf of the minor participant. I further agree to indemnify, defend, and hold harmless the Released Parties for any claims brought by or on behalf of the minor participant.
Photo and Video Release: I grant Legends Sports & Entertainment the right to use photographs or videos of myself (or my minor child) taken during participation in activities for promotional purposes without compensation. If I do not consent to this, I will notify Legends Sports & Entertainment in writing prior to participation.
Acknowledgment of Understanding: I have read and understood this General Liability Waiver and Release of Claims. I acknowledge that by signing below, I am voluntarily giving up certain legal rights, including the right to sue the Released Parties. I agree to abide by all rules and regulations of Legends Sports & Entertainment while on the premises.
